2. The application process shall consist of two phases:
   • Online submission
   • Manual submission

Before Applying Online Candidates through Bank Challan at National Bank of Pakistan Sheikh Zayed Hospital Branch Lahore Ac no= 3164498127 OR deposit the Same Amount in University Online Account PK53NBPAI707003164498127 Under Title CE UHS. The Applicant Shall enter his/ her Bank Challan on Online Application Form.
4. Application fee for
   • BPS-17 & above Rs.1000/-
   • BPS-05 to BPS-10 Rs. 300/-
   • BPS-11 to BPS-16 Rs.500/-
5. Candidates should get a print of the online form if the download option is available.
6. For Adv 01 Submit their Application Form along with attested Copies of CNIC, Domicile, Educational Documents & Passport Size Photograph at Director Admin, University of Health Science, Khayaban – e – Jamia, Punjab, Lahore.
7. For Adv 02 Submit their Application Form along with attested Copies of the following documents should submit at Assistant Registrar (Admin & Coord, University of Health Science, Khayaban – e – Jamia, Punjab, Lahore.
